The United States Health Information Knowledgebase (USHIK) is an on-line, publicly accessible registry and repository of healthcare-related metadata,
specifications, and standards. USHIK is funded and directed by the Agency for Healthcare Research and Quality (AHRQ) with management support and engagement from numerous public and private partners.
What's new in USHIK?
October 31, 2014
On October 31st a Meaningful Use issue in USHIK was corrected wherein certain Quality Data Elements (QDEs) were incorrectly linked to multiple Value Sets. Only QDEs from the April and July 2014 releases of electronic Clinical Quality Measures (eCQMs) were affected.
The affected QDEs included negation rationale attributes. For these QDEs, USHIK indicated the correct linkage to the correct Value Set as well as an extra, incorrect linkage to an additional Value Set. For example, a QDE associated with CMS2v4, "Risk Category Assessment not done: Medical reason contraindicated" was correctly linked to the "Medical reason Contraindicated" Value Set; however, the Value Set "Adult Depression Screening" was incorrectly linked to this QDE as well. This issue was present on USHIK screens and within USHIK downloads. Please note that we are not aware of any errors with eCQMs, QDEs or Value Sets in USHIK; this issue was consisted only of an extra, incorrect Value Set linkage for a particular subset of QDEs.
We believe that the issue has been completely resolved throughout USHIK, including all screens and download files. The Meaningful Use portal contains specifications, artifacts, downloads, search tools, and other resources for Meaningful Use, including Clinical Quality Measures, Value Sets, and Objectives for Stage 1 and Stage 2.
All-Payer Claims Databases
USHIK's APCD portal contains APCD File Submission Specifications from 13 states.
Several of these states contain multiple versions of their file specifications allowing a stakeholder to compare the changes that have occurred over time.

Draft Clinical Quality Measures
The Draft Measures portal in USHIK contains Clinical Quality Measures that are being made available to the public for feedback. All feedback is submitted to CMS for review.
