Select 'Death: Dead at time of assessment' only if the patient died during the stay when the incident occurred and the death was attributable, in whole or in part, to the incident. Do not make this selection if the patient is alive at the time of this report, no matter how bad the patient's prognosis. Do consider updating this report if the patient, who is alive at the time of this assessment, should die prior to discharge.
