Select 'Enteral nutritional product, including infant formula' if the nutritional product, dietary supplement or vitamins or minerals, was in a preparation intended to be administered orally as a medical food or through a nasogastric tube or a percutaneous tube placed into the stomach (gastrostomy) or into the small bowel (jejunostomy).
