Select (a)'Low risk of VTE' if the risk assessment stated that the patient was at 'low' or 'very low' risk of VTE, or if the order set, as applied to the patient, indicated that the patient did not require pharmacologic prophylaxis, or, in a patient with an increased risk of bleeding, would not have indicated pharmacologic prophylaxis even if the increased risk of bleeding had been ignored.
When the risk assessment tool used at your facility includes more than one category above 'low' such as moderate and high, or moderate, high and very high, any category above 'low' should be reported as 'high'.
