Meaningful Use Glossary

Attestation

To affirm to be correct or true, in an official capacity that the Eligible Professional or Eligible Hospital is using EHR technology in a meaningful way

Clinical Quality Measure

A clinical quality measure is a mechanism used for assessing the degree to which a provider competently and safely delivers clinical services that are appropriate for the patient in an optimal time frame.

Data Criteria (QDM Data Elements)

An atomic unit of information corresponding to data criterion in an eCQM. Defines the data type in which the element is expected to be found (including required attributes of elements), the value set names, and value set object identifiers (OID) http://www.cms.gov/QualityMeasures/01_Overview.asp

Electronic Health Record (EHR)

Allows healthcare providers to record patient information electronically instead of using paper records

Eligible Hospital

Under the Medicare and Medicaid EHR Incentive Programs, eligible hospitals and Critical Access Hospitals (CAHs) can receive incentive payments for showing that they have used certified EHR technology in ways that can positively impact patient care.

Medicare Eligible Hospitals

The following hospitals are eligible to participate in the Medicare EHR Incentive Programs:
  • "Subsection (d) hospitals" in the 50 states or DC that are paid under the Inpatient Prospective Payment System (IPPS)
  • Critical Access Hospitals (CAHs)
  • Medicare Advantage (MA-Affiliated) Hospitals

Medicaid Eligible Hospitals

The following hospitals are eligible to participate in the Medicaid EHR Incentive Programs:
  • Acute care hospitals (including CAHs and cancer hospitals) with at least 10% Medicaid patient volume
  • Children's hospitals (no Medicaid patient volume requirements)

Eligible Professional

Under the Medicare and Medicaid EHR Incentive Programs, Eligible Professionals can receive incentive payments for showing that they have used certified EHR technology in ways that can positively impact patient care.

Medicare Eligible Professionals
  • Doctor of medicine or osteopathy
  • Doctor of dental surgery of dental medicine
  • Doctor of podiatry
  • Doctor of optometry
  • Chiropractor

Medicaid Eligible Professionals

  • Physicians (primarily doctors of medicine and doctors of osteopathy)
  • Certified nurse-midwife
  • Dentist
  • Physician assistant who furnishes services in a Federally Qualified Health Center of Rural Health Clinic that is led by a physician assistant.

Meaningful Use

The American Recovery and Reinvestment Act of 2009 specifies three main components of Meaningful Use:

  • The use of a certified EHR in a meaningful manner, such as e-prescribing.
  • The use of certified EHR technology for electronic exchange of health information to improve quality of health care.
  • The use of certified EHR technology to submit clinical quality and other measures.

Simply put, "meaningful use" means providers need to show they're using certified EHR technology in ways that can be measured significantly in quality and in quantity.

Human Readable

The human-readable HTML equivalent of the XML file content.

OID

Object Identifier. A dot-delimited string which can use used to uniquely identify objects. OIDs are used within the scope of Meaningful Use to uniquely identify Value Sets. An example of an OID is "2.16.840.1.113883.3.526.3.1253" which identifies the Value Set: "Allergy to Eggs".

Supplemental Data Sets

Variables used to aggregate data into subgroups. New to 2014 measures, used to collect basic demographic data.

Value Set

A value set is comprised of code groupings drawn from multiple code systems that represent clinical concepts. Value sets provide definitions of the data elements necessary to calculate the CQM. Each value set consists of the numerical values and human-readable names, drawn from standard vocabularies such as SNOMED CT® and ICD-10-CM, that are used to define clinical concepts used in clinical quality measures (e.g., patients with diabetes, clinical visit)

Value Set Authority Center

VSAC provides downloadable access to all official versions of vocabulary value sets contained in the 2014 Clinical Quality Measures (CQMs).

XML

A CQM written in Health Quality Measures Format (HQMF) syntax. HQMF is the industry (HL7) standard for representing a CQM as an electronic document.
