Hospital Forms version 1.2 of the Common Formats is the default version displayed. To view another version, click the appropriate tab labeled with the version you wish to view.
To view a list of questions for a form, click the form name. Each question is a link you can click to see a list of the valid answers. If you wish to view more details for the form or question, click the detail link specific to what you wish to view (i.e. "View Form Detail" or "View Question Detail").
To view a list of Common Formats Forms, click the Forms link under Artifacts in the menu bar. This link will take you to a list of all Common Formats Forms currently in USHIK. From that list, you may do the following:
Shown below is an example of a comparison of three different forms. Shared attributes are highlighted in blue with a green checkmark at the left of each attribute. Unshared attributes are highlighted in grey with an exclamation point in red at the left of each attribute.
To view a list of Common Formats Data Elements, click the Data Elements link under Artifacts in the menu bar. This link will take you to a list of all the Data Elements for Common Formats that are available in USHIK. From the list of Data Elements, you may do the following:
Links for downloading Common Formats Forms or Data Elements metadata are available in either Excel or PDF formats. These download links are located at the top of Form and Data Element List Pages, and from a single Form or Data Element detail page on the "Downloads" tab.
On a list page, select the check box for Forms or Data Elements you wish to download, then select the type of download, Excel or PDF, from the buttons as shown below. The download file is a compressed (zip) file, containing an Excel spreadsheet or a PDF document, depending on the format selected.
From a Form or Data Element details page, click the "Downloads" tab and select the type of download desired.
