Status used by USHIK in the process of comparing and standardizing between data elements from different sources.
Administrative Status Date
The Administrative Status Date specifies the date the administered item attains the Administrative Status.
Answer Code provides the code used for the electronic representation of each answer value for the Common Formats data elements.
Answer ID provides the id used to represent each answer value in the Common Formats Paper Form implementation.
The set of valid values and their meanings associated with the data element. The valid values for the data element can be an enumerated list (e.g. a. Incident, b. Near Miss, c. Unsafe Condition) or non-enumerated (e.g. Patient Weight, Describe the event etc.) where the value is specified by a description (free text) provided by the user.
Code System provides the object identifier (OID) of the Code System used to represent the answer values for the data element. Example: The Code System OID for the SNOMED CT code system is 2.16.840.1.113883.6.96.
Code System Name
Code System Name provides the name of the Code System used to represent the answer values for the data element. Example SNOMED CT, LOINC etc.
The person identified by an Organization as the person responsible for submitting each Administered Item for registration.
Any general note providing additional information about the data element.
Provides the specific conditions, if applicable, under which this data element should be answered.
Data Element ID
This is the unique identifier for each data element.
Data Element ISO Name
A unique name assigned to each data element within the Common Formats. The names are developed based on the International Organization for Standardization (ISO) and International Electrotechnical Commission (IEC) 11179-5 standard.
Data Element Name (Name)
This is the unique name assigned to each data element within the Common Formats. The data element name appears as the heading for each data element in USHIK.
Data set under which this data element is categorized.
The Organization responsible for administering each Administered Item.
The data type provides information about the type of data being stored e.g. Date, Number, etc.
Date of Submission
This is date of submission of the content to the metadata registry and is auto-populated by the USHIK with the date the data is loaded into the system.
A statement that expresses the essential nature of a data element and its differentiation from all other data elements.
Description of Change
Changes to the data element and its associated metadata attributes between the current and the previous version.
End Date (Until Date)
The End Date specifies the date this version of the data element ceases to be effective.
The name of the form(s) on which this data element appears.
The manner in which data is formatted and displayed.
Provides instructions on which data element to go to next based on the response(s) selected for this data element.
Guide for Use
Instructions or advice for the interpretation, use or application of the data element.
HL7 Data Type
Provides information about the HL7 specific data type used for the data element.
The language in which the data element has been developed and is applicable.
The maximum number of storage units necessary to represent the data element value.
The text used to express the data element as a question on data collection instruments.
Rationale for Change
Provides the rationale or the reason for the change to this Common Format data element.
The Organization that is responsible for maintaining the content of the data elements (e.g. changing definitions, changing names, adding new data elements etc.) within the metadata registry.
Reference to any document(s) (including web-sites), organizations or committees from which any content of the data element originates.
The health care setting in which the data element is applicable.
The Organization that is responsible for submission of the data element to the metadata registry.
Start Date (Effective Date)
The Start Date specifies the date this version of the data element becomes effective.
An alternative name for the data element.
The Update Date specifies the date the data element metadata was last updated.
The version number of the data element.
